Unfortunately, in most cases insurance will not cover or approve labiaplasty surgery. Insurance companies look at a procedure and determine if it is “medically necessary”. They do this in a couple of different ways. One way is to define surgery as reconstructive or cosmetic. Reconstructive surgery is defined as a procedure to correct abnormal structure (like a birth defect or after a traumatic injury) to improve function or appearance. Cosmetic surgery is generally defined is the reshaping or improving of normal structures. This includes surgery done to improve the self-esteem or physical appearance of the patient. Because labia come in many different sizes and shapes, and the labiaplasty is improving the shape or appearance of a normal body part, insurance companies will almost always classify the procedure as cosmetic and not “medically necessary”.

Cases When Insurance Will Cover Procedures
Sometimes insurance companies will take into consideration the impact the issue has on daily life. For example, with breast reduction surgery having large, heavy breasts can cause ongoing upper back and neck pain. Documenting the impact of the upper back and neck pain will be one component of how the insurance company determines whether the surgery is covered or considered cosmetic.
Insurance for Labiaplasty Procedures
Because of the enlarged labia, women seeking labiaplasty complain of irritation with certain activities like riding a bike or may feel uncomfortable in tight clothing like yoga pants or swimsuits. The skin in this fragile area can be chafed and create a frequent source of discomfort. Although this can negatively affect your day to day, insurance carriers are looking for provable health issues versus a source of discomfort. While every insurance policy is different, most will still consider labiaplasty an aesthetic procedure.
So what qualifies as a “provable health issue” in the eyes of an insurer? The conditions that give patients the strongest case for medical necessity are those that have required repeated medical attention and show up in your records over time:
- Chronic or recurrent infections. If enlarged labia are contributing to repeated yeast infections, bacterial vaginosis, or urinary tract infections (UTIs) that you’ve sought treatment for, this is one of the more compelling medical necessity arguments. A documented pattern of infections carries far more weight than describing symptoms for the first time at a pre-op consultation.
- Difficulty with hygiene. When excess tissue creates overlapping skin folds that make routine hygiene difficult or cause recurring skin breakdown, some insurers may view this as a functional concern rather than a cosmetic one.
- Trauma or injury-related changes. When the labia have been structurally altered by childbirth trauma, injury, or a prior surgical procedure, the argument shifts from cosmetic improvement to reconstructive correction — a distinction that insurers treat very differently.
- Chronic pain during intercourse. Persistent dyspareunia (painful sex) that has been evaluated and documented by a gynecologist, particularly when conservative treatments haven’t resolved the issue, may support a medical necessity claim.
Even with documented conditions like these, approval is far from guaranteed. But these are the circumstances where pursuing coverage is most worth your time, and where your gynecologist and your plastic surgeon can work together to build the strongest possible case on your behalf.
How to Try to Get Labiaplasty Covered by Insurance
If you believe your situation may qualify for coverage, here are the steps worth taking before your procedure:
1. Start with your OB-GYN or primary care provider.
Your primary care doctor or gynecologist is your first advocate. Share your symptoms honestly—all of them. A documented history of visits related to your labia, whether for infections, pain, or other functional issues, is the foundation of any insurance case. Ask your provider if they would be willing to write a letter of medical necessity supporting the procedure.
2. Call your insurance provider directly.

I had had my labia removed due to fat cancer how can i get my insurance to cover it. I’m constantly getting UTIs because bacteria is easily accessible to my vagina. Most doctors I talk to don’t hear me out and it frustrating. My name is Sarah and I have read this whole page considering my current position with my labia being distinctly enlarged. This has been one of my biggest insecurities and something I struggle with everyday.
