Body Contouring After Weight Loss
When you’ve worked hard to lose significant weight, you deserve to see and feel the full results of your achievement. At Restore SD Plastic Surgery in La Jolla, Dr. Katerina Gallus specializes in post weight loss body contouring, helping San Diego area patients with skin removal surgery to eliminate excess skin after weight loss and sculpt their bodies to reveal the fit, toned figures they’ve earned.
What is Body Contouring After Weight Loss?
Body contouring after weight loss (also called post-bariatric body contouring) is a combination of surgical procedures designed to remove the loose, sagging skin after weight loss that often remains after major weight reduction—whether from bariatric surgery, medical weight loss programs, or lifestyle changes. Dr. Gallus creates customized surgical plans that address your specific concerns, often across multiple body areas.
When you lose a substantial amount of weight (typically 50+ pounds), your skin often cannot retract to match your new body size, leaving you with excess skin after weight loss that can hide your transformation and cause physical discomfort.
Treatment Areas for Removing Excess Skin Post Weight Loss
Common areas addressed with body contouring after massive weight loss include:
- Abdomen and waist
- Arms (upper arms)
- Thighs (inner and outer)
- Breasts
- Back and flanks
- Lower body (circumferential contouring)

“One of the greatest challenges after significant weight loss is the feeling of being trapped in a wrinkly lump of loose skin. Dr Gallus and her amazing team were so kind and generous in helping me peel away not only my extra skin, but in feeling like it was the right thing for me. She’s taken care of my abdomen, breasts and arms. The scars are very minimal and I feel absolutely amazing. If you’re considering plastics, you can’t go wrong with coming to her.”

Your Body Contouring Procedure Options
Body contouring after weight loss is highly individualized. Dr. Gallus will work with you to determine which combination of procedures will best address your concerns and help you achieve your aesthetic goals. Many patients benefit from combining multiple procedures in one surgery (or separating them into separate procedures if necessary for safety) to achieve comprehensive results.
Tummy Tuck (Abdominoplasty)
A tummy tuck is the most common skin removal procedure after weight loss, removing excess skin from the abdomen, tightening separated or stretched abdominal muscles (diastasis recti), and creating a firmer, flatter midsection. This is often the cornerstone procedure for post-weight loss body contouring, as the abdomen tends to accumulate significant excess skin after massive weight loss.
What it addresses: Hanging abdominal skin, weakened core muscles, stretch marks on the lower abdomen
Incision location: Across the lower abdomen, typically from hip to hip, hidden beneath underwear or bikini bottoms
Fleur-de-lis Tummy Tuck
Patients who have lost a significant amount of weight may require a more extensive body contouring procedure than a standard tummy tuck can provide. Many post-weight loss patients choose to have a fleur-de-lis tummy tuck that removes more excess skin via an additional vertical incision.
Lower Body Lift
A lower body lift (also called a belt lipectomy) is a more comprehensive skin removal surgery that addresses excess skin circumferentially around the torso—the abdomen, flanks, back, and hips. This procedure creates a 360-degree improvement and is ideal for patients with significant sagging skin after weight loss around the entire midsection.
What it addresses: Excess skin around the entire lower torso, creating a more defined waistline and lifted buttocks
Incision location: Encircling the waistline, hidden within underwear or swimwear
Arm Lift (Brachioplasty)
An arm lift removes the excess, sagging skin after weight loss that hangs from the upper arms—often called “bat wings”—creating smoother, more toned-looking arms. This skin removal procedure is particularly beneficial for patients who feel self-conscious wearing short sleeves or sleeveless clothing.
What it addresses: Loose, hanging skin on the upper arms
Incision location: Along the inside or back of the upper arm, extending from the armpit toward the elbow
Thigh Lift
A thigh lift removes excess skin from the inner and/or outer thighs, reducing chafing and creating smoother, more contoured legs. This procedure can address the inner thighs, outer thighs, or both, depending on your needs.
What it addresses: Excess skin on the inner and outer thighs, and skin folds that cause irritation
Incision location: Inner thigh incisions are typically hidden in the groin crease; outer thigh incisions may extend along the hip
Breast Lift (Mastopexy) After Weight Loss
After significant weight loss, breasts often lose volume and become deflated or saggy. A breast lift repositions the breast tissue and nipple to a more youthful position on the chest. Many San Diego breast lift patients choose to combine a breast lift with breast augmentation to restore lost volume.
What it addresses: Sagging breasts, nipple position, and/or loss of breast shape
Incision location: Varies based on technique; may be around the areola, vertically down to the breast crease, and/or along the breast crease
Combining Procedures for Comprehensive Results
Many body contouring patients benefit from combining multiple procedures to achieve their goals. Dr. Gallus carefully evaluates each patient to determine which combinations can be safely performed together, and whether procedures should be staged across multiple surgeries for optimal safety.
Common procedure combinations include:
- Tummy tuck + liposuction of the flanks and back
- Lower body lift + thigh lift
- Arm lift + breast lift
- Tummy tuck + breast lift + arm lift
Dr. Gallus will discuss the safest approach for your specific situation during your consultation.
“I went in for a consultation for a tummy tuck, bbl, and lipo and was very impressed with Dr. Gallus and her staff. She truly made me feel like she cared about my safety and would do great work to achieve the results I wanted…”

What to Expect on Surgery Day
Your body contouring surgery will be performed in our on-site, Quad A-accredited operating room, designed specifically for patient safety and comfort. You’ll be under general anesthesia, carefully monitored by our board-certified anesthesiologist, Dr. Mohamed Ibrahim.
Before your surgery begins, Dr. Gallus will meet with you to review the surgical plan and answer any final questions. The length of your surgery depends on which procedures are being performed—body contouring procedures can range from 3-6 hours or longer for extensive combinations.

Dr. Gallus uses meticulous surgical techniques to remove excess skin, tighten tissues, and create smooth, natural-looking contours. She places incisions strategically to minimize visible scarring, often hiding them in natural body creases or areas that can be concealed by clothing.
For pain management, Dr. Gallus utilizes advanced techniques including TAP blocks (transverse abdominis plane blocks) when performing abdominal procedures. This involves using ultrasound to inject local anesthetic between muscle layers, significantly reducing post-operative pain and helping you move more comfortably during recovery.
After your procedure, you’ll be placed in compression garments and moved to our recovery area, where our attentive nursing staff will monitor you closely as you wake up.
Recovery from Body Lift Surgery in San Diego
Recovery from body contouring after weight loss is a process that requires patience and commitment. The extent of your recovery depends on which procedures were performed and how many areas were treated.
What to expect during recovery:
- You will need help. Plan to have a responsible adult stay with you for at least the first few days after surgery. If you have young children, you’ll need extra help for a longer period.
- Some discomfort is normal. You’ll be prescribed pain medication to keep you comfortable. Most patients describe the discomfort as soreness rather than severe pain.
- Compression garments are essential. You’ll wear compression garments for 4-6 weeks to support your healing tissues and reduce swelling.
- Drains may be necessary. Depending on the extent of your surgery, Dr. Gallus may place temporary drainage tubes to prevent fluid accumulation. These are typically removed within 1-2 weeks during one of your follow-ups with Dr. Gallus.
- Activity restrictions are important. No heavy lifting (more than 10 lbs) for the first few weeks. Gradually return to light activities as cleared by Dr. Gallus.
- Return to work varies. Most patients can return to desk work within 2-3 weeks. Physically demanding jobs require a longer recovery period—typically 6-8 weeks.
- Exercise is gradually reintroduced. Light walking is encouraged early in recovery. More strenuous activities can typically resume after 6-8 weeks, with Dr. Gallus’s approval.
Dr. Gallus will be with you every step of the way. You’ll have multiple follow-up appointments during your recovery, and Dr. Gallus is always available to answer questions or address concerns. To enhance your recovery experience, we can arrange for you to hire a private nurse for assistance in the first few days after surgery. Some patients traveling from out of town choose to stay in the beautiful La Jolla area for their recovery period.
Body Contouring Results
You’ll see an immediate improvement in your body contours after surgery, though swelling will initially mask some of your results. As swelling subsides over the first few weeks and months, your new shape will become more apparent.
Most patients see their “final” results around 9-12 months post-surgery, once all swelling has resolved and scars have matured. The skin removal and body contouring achieved through surgery are permanent, though natural aging will continue and significant weight fluctuations can affect your results.
Maintaining your body contouring results requires:
- Maintaining a stable, healthy weight
- Following a nutritious diet
- Engaging in regular exercise
- Protecting surgical scars from sun exposure
- Following Dr. Gallus’s post-operative care instructions
